The results revealed a rare congenital anomaly: the right coronary artery originating from the left aortic sinus instead of the right, passing through an intramural course within the aortic wall. This condition causes functional compression of the artery during exertion, which can lead to myocardial ischemia and sudden cardiac death. 

Surgical Intervention 

After thorough evaluation by a multidisciplinary team of cardiac surgeons, anesthesiologists, and intensive care specialists, and obtaining informed consent, an urgent high-risk cardiac surgery was performed to reconstruct the origin of the right coronary artery and redirect its course from the aortic wall to the normal aortic root. 

By the grace of God, the procedure was completed successfully. The patient regained consciousness immediately without complications and was discharged on the fifth day post-surgery in stable condition.
