The medical team at Al-Mouwasat Hospital in Madinah, thanks to God, was able to end the suffering of a patient in his seventh decade who suffers from complete paralysis of the lower extremities and lack of control over the excretory process.

The patient was brought to the orthopedic and spine clinic at Al-Mouwasat Hospital - Madinah to review Dr. Ahmed Khamis, consultant orthopedic and spine surgery, and by clinical examination and magnetic resonance imaging, it was found that there was instability in the lumbar vertebrae with compression of the spinal cord at several levels, which led to his complete paralysis in the lower limbs and disrupt some vital functions.

The doctor explained the situation to the patient and his family and advised them to perform an urgent surgery to remove the pressure on the spinal cord and stabilize the vertebrae with the placement of stents and metal plates between the vertebrae, so that he regains his ability to move normally and lead a normal life.

On the next day, the patient was examined after his approval by the anesthesia consultant to ensure his eligibility for surgery and the necessary tests.

After the patient entered the operating room, and using the latest microscopic techniques, the consultant removed the compression on the spinal cord, stents were installed between the vertebrae and then fixed using stem screws and metal poles.

 Then the patient was transferred to a special room to receive full health care and to check on his condition and follow him closely for three days.

The patient left the hospital on foot after God blessed him with regaining his ability to move and excrete normally.

Comprehensive Case Evaluation 

The patient was referred from another hospital for feeding tube placement. Upon arrival, her case was immediately reviewed by a multidisciplinary medical committee led by Dr. Ahmed Al-Husseini, Consultant of General Surgery and Oncology, with participation from specialists in gastroenterology, endoscopy, diagnostic radiology, and oncology

After conducting essential investigations, including multi-slice CT scans and endoscopic biopsy, the team decided on urgent surgical intervention to remove the tumor.

The complex surgery involved: 

  • Radical resection of approximately 85% of the stomach. 
  • Complete removal of surrounding lymph nodes and peritoneal membranes to ensure no cancer cells remained, confirmed later by histopathology. 
  • Creation of a gastrointestinal anastomosis to connect the remaining stomach to the intestines, restoring the patient’s ability to eat. 

Recovery after Gastrectomy 

Following five days of intensive care, the patient gradually regained strength and was able to eat orally for the first time in over a month—a milestone in her recovery journey. She was discharged in excellent health, marking a significant success for the surgical team.

Case Details 

A 41-year-old patient presented with chronic pain in the right leg, numbness, and altered sensation in the sole of the foot for over five years. The pain worsened with pressure or touch and intensified at night, causing severe insomnia. Despite consulting multiple physicians, the condition remained undiagnosed. 

Accurate Diagnosis 

Clinical examination and advanced imaging (X-ray and MRI) revealed a Tibial Nerve Schwannoma, a benign nerve sheath tumor located in the posterior compartment of the leg. This nerve controls foot movement and sole sensation. Continuous pressure from the tumor posed a risk of loss of sensation or motor weakness in the foot and toes. 

What is Tibial Nerve Schwannoma? 

A Tibial Nerve Schwannoma is a rare, benign tumor originating from Schwann cells that form the nerve sheath. It affects the tibial nerve, which is responsible for foot movement and sensation in the sole. While the tumor grows slowly, its enlargement can compress nerve fibers, leading to: 

  • Chronic leg pain 
  • Numbness or loss of sensation in the sole 
  • Weakness in foot and toe movement 

Microsurgical Tumor Removal 

Dr. El Shazly successfully performed a microsurgical excision of the tumor (5×3 cm) while preserving nerve fibers. The patient was discharged the next day without complications and experienced significant pain relief and restored sensation within days. Histopathology confirmed the complete removal of the schwannoma.
