The medical team at Al-Mouwasat Hospital in Qatif succeeded in ending the suffering of a patient in her seventies who was suffering from severe pain that impeded the movement of the left knee and completely disrupted the function of the left side due to advanced roughness in the knee joint, by performing a complete replacement of the affected knee joint.

The patient stated that she went to Al-Mouwasat Hospital after undergoing a trial of treatment by injecting the knee, and there was no response.

After the clinical examination, normal x-rays were taken on the knees and it was found that they suffer from advanced friction and roughness in the left knee joint that leads to severe pain during movement and sitting, and erosion of the left knee cartilage, and erosion of the joint surface.

The patient was directed by a consultant orthopedic surgeon, Dr. Thomas Papageorgiou performs a complete knee joint replacement so that the patient can move normally, the pain will go away and the left side will return to its normal state. Indeed, a complete knee joint replacement was performed with a high-quality artificial joint through a small opening in the front of the left knee.

After the operation, the patient was transferred to the recovery room and she is in a very good health condition, as it was confirmed that the blood circulation to the left lower extremity was working efficiently, and it was found the next day that the patient did not suffer from any pain after the operation, thanks to the anesthesia catheter, and she also did not need To long-term doses of antibiotic after the operation, which indicates the level of super sterilization inside the operating room, the patient was able to walk on the second day of the operation normally and left the hospital in a good health condition to practice her daily life normally

Case Details 

A 41-year-old patient presented with chronic pain in the right leg, numbness, and altered sensation in the sole of the foot for over five years. The pain worsened with pressure or touch and intensified at night, causing severe insomnia. Despite consulting multiple physicians, the condition remained undiagnosed. 

Accurate Diagnosis 

Clinical examination and advanced imaging (X-ray and MRI) revealed a Tibial Nerve Schwannoma, a benign nerve sheath tumor located in the posterior compartment of the leg. This nerve controls foot movement and sole sensation. Continuous pressure from the tumor posed a risk of loss of sensation or motor weakness in the foot and toes. 

What is Tibial Nerve Schwannoma? 

A Tibial Nerve Schwannoma is a rare, benign tumor originating from Schwann cells that form the nerve sheath. It affects the tibial nerve, which is responsible for foot movement and sensation in the sole. While the tumor grows slowly, its enlargement can compress nerve fibers, leading to: 

  • Chronic leg pain 
  • Numbness or loss of sensation in the sole 
  • Weakness in foot and toe movement 

Microsurgical Tumor Removal 

Dr. El Shazly successfully performed a microsurgical excision of the tumor (5×3 cm) while preserving nerve fibers. The patient was discharged the next day without complications and experienced significant pain relief and restored sensation within days. Histopathology confirmed the complete removal of the schwannoma.
